This was to ensure that the eligibility pathway had been followed in line with the National CHC Framework.
We were required to review 121 cases eligible for jointly funded support during a defined project period of 14 weeks, ensuring that each individual was central to any conversations concerning the CHC process.
Recommendations were made by the Liaison Care team based on the review discussion, joint funding tool and established health needs, if present.
Using the calculations within the joint funding tool provided by the ICB, and assuming that the costs recorded on Broadcare remain accurate, accepting the recommendations made by Liaison Care’s review would enable a saving of over £4,000,000.
Although the remaining 62 cases had been through the framework at some stage, they had not been reviewed in the past 12 months, so changes in circumstances had not been take into consideration.
In conclusion, there were 43 cases assessed, resulting in a reduction in joint funding percentage responsibility for health, and 42 cases deemed to have no eligibility for joint funding to continue. The remaining 2 cases were found to be eligible for both CHC funding and FNC.
