At Liaison Care, we’re here to support you – whether you’re a patient, a family member, or a representative. We offer a range of health and social care services across the country, working with NHS Integrated Care Boards (ICBs), Health Boards (HBs), and Local Authorities (LAs) to complete assessments and reviews that focus on your needs and aspirations.
We know that every person is unique, which is why we put you at the heart of everything we do. Your voice matters – and so do the views of your family or representative. We’ll listen carefully, record your views clearly, and make sure they are fully considered throughout the process.
From the first point of contact to the final review, our experienced team of nurses and social workers will be with you every step of the way. We understand that this process can feel overwhelming, and we’re here to answer your questions and provide reassurance whenever you need it.

How We Can Help
Completion of Continuing Healthcare (CHC) Checklists, Decision Support Tools and CHC 3- and 12-month reviews.
